Appointments should be scheduled by calling (910) 893-1560. Please bring your student ID and insurance card with you. All services to be filed with insurance.

Before your appointment at the Health Center completed and submit one of the following forms, per your appointment type:
- Standard New Patient Form (Family Medicine, OMM, Sports Medicine)
- New Patients Weight and Wellness Management Form:

Immunizations
All students entering any North Carolina four-year college or university are required by state law to show proof of certain immunization. This applies to all students who meet one or more of the following criteria:
- Enrolled in on-campus, in-person courses
- Taking any class of four or more credit hours
- Living in an on-campus residence facility
- Taking any courses that meet during the day
Students who meet any of these criteria must submit a Certificate of Immunization or Official Immunization record must be presented on or before the date the person first registers for a semester.
