Student ID number:   Your studentID number is located on your Student ID card.  If you do not have a student ID card, you can also locate the number on most computer generated statements and/or grade reports you receive from the university.  If you are a student at one of the extended campuses, you may call the office at your campus to obtain your student  ID number.  Your student ID number IS NOT the same as your social security number.  For your safety DO NOT enter your social security number in this form.
