This form gives authorization to transfer funds from a student account to a camel card.
Refund Request Form
This form allows you to request a refund of credit from a student account. Effective fall 2011 semester, students will be required to enroll in e-refunding to receive a refund of student account credit balances. As of August 1, 2011, the Refund Request Form will no longer be available. Please visit the Refund Information section of the website for additional information.
Change of Billing Address Form
Use this form to update a billing address.
Parent Loan-Authorization to Release Funds
Federal Parent PLUS Loan borrowers may use this form to request a release of loan funds to a student.

This form allows Parents to request a paper check refund of a credit balance created by a Parent PLUS Loan.
