This includes submitting three letters of recommendation, a personal statement, a recent photo, the PT observation hours form, and $50 application fee. - Submit all official college transcripts to the CPHS Admissions Office.
- Submit GRE score report. Applicants are required to send an official GRE score report directly to the CPHS Admissions Office using the code 4575.
The PT Observation Hours Form can also be mailed to the CPHS Admissions Office if applicants are not able to scan and upload the form to the online application.
