The undersigned Intern, Internship Supervisor, and Internship Host agree that this contract, and its attachments, establish the minimum requirements for the Intern’s successful completion of a Campbell University Internship and the Internship Supervisor’s award of a letter grade for up to three hours of academic credit. 

I.       Intern and Faculty Internship Supervisor agree that: 

  1. The filed copies of the Independent Study Contract and the Statement of  Objectives and other items as specified below, form part of this contract.  
     
  2. The Intern is eligible to earn up to three hours of academic credit, the amount to be determined by the Internship Supervisor, for successful completion of an internship under the terms of this contract. 
     
  3. Upon enrollment, the Intern will pay for three credit hours at the rate for credits earned through regular course work at Campbell University. 
     
  4. The Campbell University grading system shall apply to the Internship, and the Internship Supervisor shall determine a grade based upon the following
    1. The Internship Paper (65 %) – a formal written report of no less than 25 pages on a topic to be approved by the Internship Supervisor and related to the substance of the internship.  (Without approval of some later date, the Paper is due to the Internship Supervisor at the completion of the Internship).  See attached paper guidelines and citation format.
    2. The Intern’s Journal (10 %) – a daily journal summarizing the Intern’ s principal work activities and relating them to the internship’s objectives, previous academic work, or career expectations.  (The Intern must present the Journal for the supervisor’s examination during the interviews scheduled at mid-term and the end of the Internship).
    3. The Host Agency’s Evaluation(s) (20 %) – given directly by the Host to the faculty Internship Supervisor.
    4.  Oral Interviews (5 %) – The Internship Supervisor will evaluate the Intern’s progress and accomplishment of course objectives via an oral interview during the mid-point and completion of the internship.
    5. Completion of Specified Hours – Students must successfully complete the specified minimum number of hours and weeks: Summer Internships: 120 hours over an 8 week period; Fall or Spring Semester: 150 hours over a 12 week period.
  5. With the exception noted hereafter, the Intern also agrees to initiate a meeting with the Internship Supervisor, when necessary, in order that the Intern’s progress might be reviewed.  (For internships conducted during the summer at a distance from Buies Creek, telephone conferences or letters in accord with a schedule developed by the Intern, and approved by the Supervisor, and included as an attachment to this contract may substitute). 
     
  6. The Internship Supervisor agrees to contact the Host twice during the period of the internship and to obtain from the Host, both at the midpoint and at the conclusion of the internship, assessments of the Intern’s contribution to the Host’s organization. 
     
  7. The Intern will received a copy of this Contract and attending documentation.
     

II.    Intern and Internship Host Agency agree that:     

  1. During the period from _____________________ to ____________________, the Host will provide the Intern an opportunity to learn through working at the Host’s office and under his/her, or an appointee’s direction and supervision.  Appropriate tasks  for the Intern are helping the Host with research, support, or general assistance for approximately 150 hours (Summer Intern-ship: 120 hours over an 8 week period). 
     
  2. The Host will provide the Intern with office space, equipment, secretarial services, supplies, and such other support as is appropriate for accomplishing   the Intern’s assigned tasks.
     
  3. From time to time during that period, the Host, or an appointee, will provide  informal counseling on the Intern’s performance of tasks and will, for a half hour at least once each week, confer formally with the Intern regarding the progress of the internship.  At the conclusion of the internship, the Host will provide the Internship Supervisor with an evaluation of the Intern’s contribution to the Host’s office.
     
  4. The internship is as much an educational endeavor as a work experience.  The Intern consequently is not a regular employee of the Host nor eligible for such benefits as contributions to retirement pay, sick leave, paid vacation, or workmen’s compensation, but may receive a stipend or pay as agreed upon.  
     
  5. The Intern may, on an occasion, be absent from the Host’s office as necessary to attend classes at Campbell University and participate in conferences related to the internship, when the internship is conducted close to the University.

III.    Faculty Internship Supervisor and Internship Host Agency agree that: 

  1. The Host will provide the student with a work place and job experience useful to the student’s academic development while working under the Host’s supervision.
     
  2. To ensure the student achieves his/her potential for learning during the internship, the Host agrees to make available a quality work area, proper materials and equipment, and an experienced supervisor who will oversee and guide the Intern through the internship effort.
     
  3. The Host will complete and return a final student assessment within one week of receipt of the evaluation form.
     
  4. The Host will contact the Internship Supervisor immediately should problems arise or should questions occur regarding the Intern - Host relationship.
